New Applicants

Upload evidence of 50 ACCME-accredited CMEs completed within the past 24 months from application date.
If any initial board certification(s) have lapsed, upload evidence of 100 ACCME-accredited CMEs completed within the past 24 months from application date.

Renewing Applicants

Upload evidence of 50 ACCME-accredited CMEs completed within the past 24 months from previous certification date.

If your NBPAS certification has lapsed more than 6 months and your initial board certification is also lapsed, upload evidence of 100 ACCME-accredited CMEs completed within the past 24 months from application date.

Acceptable CME Documentation
  • AMA Category 1 credits, or AOA Category 1A/2A credits only
  • PDF or JPEG, TIF, PNG (i.e. photo from iPhone) formats only (no Word, Excel, or self-maintained documents)
  • Clearly state name of applicant, name of ACCME accredited provider, date of completion, number & type of CME hours earned
  • Please note: Maintenance of Certification (MOC) points will not be accepted, as MOC points are not always equivalent on a 1:1 basis for CME credits.

Fellows/physicians-in-training: In lieu of CME evidence, upload a copy of your fellowship certificate or a letter from your program director indicating that you are/have participated in an ACGME-credentialed training program within the past two years.

Military Applicants: Please upload evidence of military enrollment (i.e. DD214, VA card, current mil.com email, or enlistment certificate).
